How do I schedule an appointment?
Insurance companies
usually require a written referral from your primary care physician
before an orthopedic specialist can see you. Without a referral
your insurance company may not pay for your visit and/or reimburse
at a much lower rate. You should contact your insurance carrier
to see if our providers are covered under your insurance plan, their
contact phone number is usually located on your insurance card.
Once a referral
is obtained, or your insurance company has verified that you do
not need a referral, you may call us at 503-639-6002
or toll free 1-877-999-6002 to schedule your appointment.
How do I know if Orthopedics Northwest is contracted
with my insurance?
We are contracted
with many insurance companies. To make absolutely certain that we
are on your insurance plan, you need to call your insurance for
verification. They will ask for the information on your insurance
card as well as what physician you plan to see. Please give them
our clinic name, Orthopedics Northwest, and the names of our
physicians (Dr. Scott Grewe, Dr. Gregory Irvine, Dr. Stephan Prô, and Dr. Robert Wilson).
In addition, our certified physicians assistants are: Scott Beyer,
PAC; and Bill Stahlberg, PAC.
What do I need to bring with me when I come
in for my appointment?
Referral:
Our office will need a referral from your primary care physician,
if your insurance company requires one. Without a referral your
insurance company may not pay for your visit and/or may reimburse
at a much lower rate, leaving you with a much higher out of pocket
expense. Please call your insurance company to verify if you need
a referral.

Diagnostic
studies: X-rays, MRI studies, and any other studies you
may have had performed that pertain to your current orthopedic condition.
Reviewing x-rays and MRI’s are an important part of your visit
with the doctor. It is best to hand carry the films with you rather
than to have them mailed that way it assures they will arrive in
time for your appointment. The facility that performed the studies
will either release the films to you or make a copy for you. Please
be sure to contact them ahead of time so they can accommodate your
needs for your office visit.
Your insurance card: Your insurance card provides important
detailed information we need for billing the insurance company for
your services.
Photo ID: We ask that you bring a photo ID so we can verify you are the correct patient.
Co-payment:
We ask that you pay your co-payment at the time of your visit. The
co-payment amount is determined by your insurance plan and is usually
listed on your insurance card.

Patient Profile form: We usually mail this form to you
ahead of time so you can complete it at your leisure before your
office visit. This form can also be printed from this website or
you can request it to be sent via US mail. Please complete this
form as it will provide us with accurate contact and insurance information.

HIPAA & RED FLAG Rule: Orthopedics Northwest is
now required by the Federal Trade Commission to show a copy of a
valid Photo ID from each patient or Patient’s Guardian presenting
to our office. If you are unable to provide a valid Photo ID, we
may not bill your insurance company, and you may have to pay for
your visit at the time of service.
